NATIONAL DIABETES AUDIT (NDA)

This GP Practice is taking part in an important national project about diabetes care and treatment in the NHS in June and July.  The Project is called the National Diabetes Audit (NDA).  To take part, your GP practice will share information about your diabetes care and treatment with the NDA. The type of information, and how it is shared, is controlled by law and enforced by strict rules of confidentiality and security. 

If you do not want your information to be used, please inform the receptionist, your GP or nurse. This will not effect your care. 

COMMUNICATION AND INFORMATION

The Practice is improving how we communicate with patients.  Please tell us if you need information in a different format or require communication support.

TARGET TRAINING DAY

The surgery will be CLOSED for training purposes from 12 noon on Thursday 28th September 2017. If you need a doctor urgently whilst we are closed please ring NHS 111.  In an emergency telephone 999 for an ambulance.  The surgery will re-open at 8 am on Friday morning.

ON-LINE APPOINTMENTS & PRESCRIPTIONS!! - You can book your appointment on-line, up to 3 months in advance & also request repeat prescriptions.  Ask at reception for a password.  All appointments with the doctors are 10 minutes, if you have several problems to discuss please ask for a longer appointment. 

CHLAMYDIA Testing for males & females aged 16-25.

Chlamydia is a sexually transmitted infection that often has no symptoms.

The test is a simple, painless, self taken swab for females and a urine test for men - ask your doctor, nurse or receptionist for a testing kit.

Why should I be tested?

To stop the spread of infection & prevent fertility problems in the future.  Every time  you change your sexual partner you should get a test

The treatment is a short course of strong antibiotics.

FREE CONDOMS

If you are over 16 you can collect free condoms from the surgery. 

Ask your doctor, nurse, or receptionist.

You can ring the receptionist and arrange to collect your condoms from reception.

If you are aged 13-16 you can collect free condoms from the surgery - please contact the surgery to arrange with the Health Care Assistant.
